It depends on your needs. Like the other answers, Adobe Acrobat Professional is arguably the most flexible solution.

However, if you need to be able to have all the editing options available to you for much less money ($99), try Nitro Pdf:

http://www.nitropdf.com/professional/overview.htm

Also, if you need free pdf annotation tools that are on par with Acrobat Professional, try PDF-XChange viewer:

http://www.docu-track.com/home/prod_user/PDF-XChange_Tools/pdfx_viewer

You do not even have to install this - the portable version can be unzipped and started.

To really make some (graphic) changes to a pdf, Illustrator can be used as well.

You need full Acrobat Pro. Reader is just that: a free ap for reading PDFs. Acrobat Pro is the one that lets you edit and otherwise modify pdfs.

You can create overlays to PDF files with PDF Xchange. It's a free download. To actually edit the text, you'll need to spend money, and there are a lot of shareware tools out there. One "free" way to do it is to print the document, scan it, OCR it and then edit the resulting text - but it's only free if you have access to a scanner.
